Insurance Benefits

1. Death Risk Benefits
• If the Insured Member gets Accident since the first month and so forth that resulting in Death, then the Manager will pay Insurance Compensation and Total Balance (if any) to the Insured Member account in the Policy Holder.
• If the Insured Member death is not caused by an accident, but due to natural cause or sickness, since the 13th (thirteen) month and so forth, then the Manager will pay Insurance Compensation and Total Balance (if any) to the Insured Member account in the Policy Holder.

2. Total Permanent Disability Benefits
• If the Insured Member gets Accident since the first month and so forth that resulting in Total Permanent Disability, then the Manager will pay Insurance Compensation and Total Balance (if any) to the Insured Member account in the Policy Holder.
• If the Insured Member Total Permanent Disability is not caused by an accident, but due to natural cause or sickness, since the 13th (thirteen) month and so forth, then the Manager will pay Insurance Compensation and Total Balance (if any) to the Insured Member account in the Policy Holder.

3. End Period of Membership Benefits
• If there is no benefit received up to the End Period of Membership, then the Manager will pay End Period of Membership Benefits to the Insured Member account in the Policy Holder.

If  Mr Danar is dead/having Total Permanent Disability on his 38 years of age, the Insurance Benefit that he will receive is Insurance Compensation for Rp 50.000.000,- and Total Balance (if any).

1.    If Mr Danar lives until the end of Membership Period (40 years old) and never being late on Contribution payment, then Mr Danar will get End Period of Membership Benefit.
2.    If the Accumulated Balance is Rp 16.525.000 (greater than the total contribution that Mr Danar has paid), then the End of Membership Period Benefits that will be received is Rp 16.525.000.
3.    If the Accumulated Balance is Rp 10.750.000 (smaller than the total contribution that Mr Danar has paid), then the End of Membership Period Benefits that will be received is Rp 10.750.000.

Grace Period and Lapsing Membership
1.    The Manager will give Grace Period of contribution payment for 3 (three) consecutive days of due date, counted since the first due date of contribution payment.
2.    The Manager will return the excess of Total Balance (if any) if it is more than Rp 50.000,- to the Insured Member when the membership is lapsing.

Fee

Prima Amanah Insurance Contribution is including the administration fee (Ujrah), Insurance fee (Tabarru’), Bank Commission and Marketing Fee (if any).

Risks

1. Membership will lapse and the Sharia Life Insurance will be invalid if the Contribution is failed to be paid until the end of Grace Period and the Total Balance is insufficient to perform payment for Tabarru’ and Ujrah.
2. The benefits will not be paid off the Insured Member is dead and/or having a total permanent disability as a result of this following causes:

• Pre-existing condition, except if it is passed 24 (twenty-four) months since the valid date of Membership or, the updated issue date of Insured Membership recovery or updated addendum date (the least date);
• Suicide or suicidal attempt and/or self-hurting in healthy or unhealthy mental condition;
• Act of violating law or crime that is performed by the Insured Member, or participation of the Insured Member in a riot, fight, or any kind;
• Law violation that is performed by the Beneficiary or with the help of someone who concerns with the Sharia Life Insurance;
• Natural disaster or epidemic disease that is declared by the government;
• War, invasion, foreign enemy action or any other war (both declared or undeclared war) that is involving Indonesia;
• Insured Member activities that are related to the duty in army from any country or any authority;
• Human Immuno-Deficiency Virus (HIV) or Acquired Immune Deficiency Syndrome (AIDS) disease or condition related to HIV/AIDS;
• Dangerous activities and/or risky activities, including but not limiting on performing car/motorcycle race, diving (or any other underwater activities), wall climbing/mountain climbing, sky-diving, boxing, professional sport, winter sport and riding horse;
• Involving in flights except as a passenger in regular commercial flights;
• Consumption of alcohol, prohibited substances, poison, etc.;
• Nuclear reaction, radiation, or radioactive contamination;
• Death and/or Total Permanent Disability in the first 12 months of Membership, except if the Insured Member is dead and/or having a total permanent disability that is caused by accident.

Requirements and Procedures

1. Contribution Payment, Contribution can be paid monthly and will be claimed by the Insured Member when the Contribution is received in the Manager account;
2. Benefit Claim Application, Claim can be applied to the Manager in written form within 90 (ninety) calendar days since the Insured Member is dead/having total permanent disability attaching real documents needed that noted in Insurance Certificate.
3. Document Completion, documents that have to be prepared by the Insured Member when registering the program.

New Membership of Prima Amanah Insurance

1. Registration form that is completed and signed by the Insured Candidate, and signed by Marketer.
2. Copy of ID card; and
3. Other documents (if necessary).

Documents needed as claim application requirements

1. Claim form that is filled well and complete;
2. Real Insurance Certificate;
3. A validated copy of the Insured Member and Beneficiary’s ID card;
4. Medical Certificate (if the Insured Member is having Total Permanent Disability);
5. Death Certificate (if the Insured Member is passed away);
6. Additional documents (if necessary).
